Health and Immunization Information
Incoming Student Health Insurance
The University of Chicago requires that all students carry health insurance. All students will automatically be enrolled in the University's Basic Insurance plan on September 1, 2008. If you will plan to have insurance under another carrier, you will need to waive your University insurance by October 10, 2008. Students who wish to upgrade or waive their health insurance may do so through the Student Health Enrollments page.
Early Enrollment Option
Incoming students whose existing health insurance expires before September 1 are able to enroll in a one-month basic health insurance plan through the University of Chicago. This plan offers the school's Basic Plan coverage from August 1 - August 31, 2008.To participate in the Early Enrollment plan, fill out the Early Enrollment Form and mail it, along with the appropriate premium, to the Dallas, TX address. - Note: Only basic coverage is available for the Early Enrollment Option. Students are eligible to upgrade their University insurance to the Prescription Advantage Plan by visiting the student enrollments page. However, this upgrade will not go into effect until September 1, 2008.
Required Immunizations for Incoming Students
Full-time incoming medical students must present proof of immunity to the following by July 1, 2008:
- proof of immunity through blood titer to Measles (Rubella), Mumps, German Measles (Rubella) and Hepatitis B;
- proof of immunity through blood titer or vaccination to Varicella;
- up-to-date Tetanus/Diphtheria vaccination;
- screening for exposure to tuberculosis;
Failure to submit proof of immunization or vaccination will result in class registration penalties and reduced access to campus facilities (library, athletics, etc.) by the University Registrar.

Forms must be completed by a Health Care provider or you may attach photocopies of your childhood immunizations that were signed by a licensed provider.
